WhatsApp appointment reminders for clinics in India: timing, template examples, Meta rules
Short answer: Send a WhatsApp confirmation when the visit is booked and a reminder before it, using Meta-approved utility templates. Reminders and promotions go in separate templates. Every reminder offers YES, a reschedule option and CANCEL, and STOP works as the opt-out. Only message patients who opted in, and keep health details out of the message.
If you'd rather not build this yourself, Clinic Connect sends these reminders from the clinic's own WhatsApp number.
When to send each message
We haven't found a published study that sets the right reminder times for clinics in India. So this table uses only two kinds of source: Meta's rules, and the defaults ChikuOS Clinic Connect uses. Choose times that match how far ahead your patients book.
| Moment | Message | Why then | Source |
|---|---|---|---|
| When the visit is booked | Booking confirmation, with YES to confirm | Meta's utility examples include confirming an order or transaction the person made | Meta template categorization (checked Oct 2026) |
| About 24 hours before | Reminder with Confirm, Reschedule and Cancel buttons | Leaves the front desk a day to refill a slot | Clinic Connect setting (default 24 hours) |
| A few hours before, on the day | Short same-day note | A late nudge for the same day | Clinic Connect setting (default 3 hours; each clinic can change it) |
| After the patient replies | A free-form reply from reception | A patient message opens a 24-hour customer service window | WhatsApp Business Messaging Policy, s.2 (checked Oct 2026) |
| More than 24 hours after the patient's last message | Approved templates only | Outside the window, only approved templates may be sent | WhatsApp Business Messaging Policy, s.2 (checked Oct 2026) |
| Late at night | Nothing. Hold the reminder | The clinic sets quiet hours, and reminders due in them are skipped | Clinic Connect behaviour |

7 template examples (demo data)
These use made-up data: Demo Clinic, patient Asha (demo), Tuesday at 4:30 PM. {{1}}-style placeholders are template variables. Each template goes to Meta for review. These examples are not pre-approved.
- Booking confirmation (Utility)
Hello {{1}}, your visit at {{2}} is booked for {{3}}. Reply YES to confirm. To cancel this visit, reply CANCEL.Example: Hello Asha, your visit at Demo Clinic is booked for Tue, 4:30 PM. Reply YES to confirm. To cancel this visit, reply CANCEL. - Visit reminder (Utility), with buttons Confirm / Reschedule / Cancel. This is the wording in the screenshot below.
Hello {{1}}, this is {{2}}. Your visit is at {{3}}. Use a button below to confirm, reschedule, or cancel. - Same-day reminder (Utility)
Hello {{1}}, your visit at {{2}} is today at {{3}}. Reply if you're running late. - Reschedule request received. This is sent inside the 24-hour window, so it doesn't need a template.
Thanks, {{1}}. Reception will message you with a new time for your visit. - Cancellation confirmed (Utility)
Hello {{1}}, your visit at {{2}} on {{3}} is cancelled. To book again, reply to this message. - Next session due (Utility, only for a plan the patient has already booked)
Hello {{1}}, your next {{2}} session at {{3}} is coming up. - Opt-out confirmed (Utility)
You won't get promotional messages from {{1}} on WhatsApp now. Visit updates will continue. Reply STOP ALL to stop everything.

CANCEL and STOP are different words. CANCEL cancels one visit. It isn't an opt-out. STOP is the opt-out: in Clinic Connect, STOP ends promotional and package messages but visit updates continue, STOP ALL ends everything, and START turns promotional messages back on.

Meta's rules, checked Oct 2026
- Template categories. Meta has three categories: marketing, utility and authentication. A utility template must be non-promotional, and it must also be either specific to the user (their order, account or transaction) or essential to them. Source: Meta, Template categorization (page updated 15 Sep 2026).
- Utility vs marketing. Meta treats a template that mixes utility and promotional content as marketing. The same goes for a template whose content is unclear, such as one that is only "{{1}}". If you submit a template as utility and WhatsApp decides it's marketing, it's approved as marketing. Meta can also recategorize templates that are already approved. A business that keeps mislabelling marketing as utility can have its utility messages limited. Source: same page.
- Opt-in. You may message someone only if they gave you their number and opted in to hear from you. You choose how to collect opt-in and must make sure it meets the law. You must honour any request to stop, made on or off WhatsApp. Meta suggests a separate opt-in for each category of message. Source: WhatsApp Business Messaging Policy, s.1 and its opt-in guidance (last updated 23 Sep 2026).
- 24-hour window. Business-started conversations need an approved template. You may reply without a template within 24 hours of the patient's last message, and each new patient message restarts that 24 hours. Source: same policy, s.2.
- Health information. The policy says not to send or request health information where the rules require stronger handling than WhatsApp provides. Keep reminders to the time, the clinic and the visit type. Source: same policy, s.3.
Consent and deletion under India's DPDP Act
This is general information, not legal advice. Ask your own lawyer about your clinic.
- Consent: From 13 May 2027, clinics must get consent that is "free, specific, informed, unconditional and unambiguous with a clear affirmative action", and limited to the data the stated purpose needs. Source: Digital Personal Data Protection Act, 2023, s.6(1).
- Language: From 13 May 2027, clinics must present each consent request in clear and plain language, with the option for the patient to read it in English or a language in the Eighth Schedule. Source: s.6(3).
- Withdrawal: From 13 May 2027, clinics must let a patient withdraw consent at any time, as easily as they gave it. Source: s.6(4).
- Erasure: From 13 May 2027, unless another law requires them to keep it, clinics must erase personal data when the patient withdraws consent, or once the purpose is no longer served. Source: s.8(7). From the same date, patients have a right to correction and erasure. Source: s.12.
- Timing: The government notified the DPDP Rules, 2025 on 13 Nov 2025 (G.S.R. 846(E)). They come into force in stages. Rule 4 starts one year after notification. Rules 3, 5 to 16, 22 and 23 start eighteen months after, on 13 May 2027. Most Act duties described above (ss.6(1)-(8), 8 and 12) also start eighteen months after the 13 Nov 2025 notification (G.S.R. 843(E)). Sources: DPDP Rules, 2025, r.1; MeitY notification G.S.R. 843(E).
How Clinic Connect handles deletion: Clinic staff can erase a patient's WhatsApp data on request from the patient's page. Their number is removed from message history and reminders stop until the patient opts in again.
Common reasons templates get rejected or recategorized
- Mixing an offer into a reminder. For example: "See you tomorrow. Also, ask about our new package." Meta treats mixed content as marketing.
- Vague bodies, such as "{{1}}" alone or "Congratulations!". Meta classes unclear content as marketing.
- Choosing utility for a broadcast. A general health tip sent to everyone isn't specific to one patient's visit.
- Putting health details in the message. A diagnosis, test result or treatment has no place in a reminder. Name the visit, not the condition.
- Unclear variables. Meta requires an example value for each variable when you create a template. Use realistic samples, such as "Asha" and "Tue, 4:30 PM", so reviewers can see what each
{{n}}will hold. Source: Meta, Template fundamentals (updated 21 May 2026). - Telling patients STOP cancels their visit, or that CANCEL opts them out. Keep the two words apart.
FAQs
Do clinics need an approved template to send WhatsApp appointment reminders?
Yes. A reminder starts the conversation from the clinic's side, so it needs an approved template. Reception can reply without a template only within 24 hours of the patient's last message.
Is an appointment reminder a utility or a marketing template?
A plain reminder about a visit the patient booked fits Meta's utility criteria: it's non-promotional and specific to that patient. If you add an offer, Meta treats it as marketing.
What should happen when a patient replies CANCEL?
That visit is cancelled, or flagged for reception to cancel, and its later reminders stop. CANCEL doesn't opt the patient out of WhatsApp. STOP does that.
Can reminders include test results or a diagnosis?
We advise against it. Meta's policy restricts health information where the rules require stronger handling. Keep reminders to the time, the clinic and the visit type.
Do reminders reduce no-shows?
We don't publish a no-show figure, and we can't promise any change. A clear reminder with an easy way to confirm, reschedule or cancel gives the front desk more notice when a slot opens up.
